Innovation on Purpose: Telehealth

What Sparked It?

Rather than treating virtual care as a stopgap, leaders in Illawarra Shoalhaven LHD asked what it might look like to proactively support people living with chronic conditions at home.

What if we could support complex, at‑risk patients at home – not in hospital?

Where Curiosity Met Context

In Illawarra Shoalhaven Local Health District (ISLHD), the VeCC service launched in 2019 as part of NSW Health’s Virtual Care Accelerator.

ISLHD faced rising chronic disease burden, emergency presentations, and hospital stay pressures. They sought a digitally-enabled alternative that improved outcomes and reduced admissions.

Chasing the Unknown

With the question as their compass, the team co-designed a multidisciplinary, home‑based model with digital support:

  • Stream 1: ED-to-community care coordination, navigation, and coaching for high-use patients
  • Stream 2: Chronic disease management with remote monitoring (blood pressure, glucose, oxygen, weight) plus virtual check-ins
  • Stream 3: Virtual Hospital Ward (VHW): patients at risk of hospitalisation receive clinical oversight at home up to 3× daily, supported by real-time monitoring devices.

The initiative was built from lean experimentation, allowing iteration across digital and care-delivery teams.

What Happened Next?

The VeCC service reduced the burden of treatment for patients and lowered avoidable hospital admissions.

In 2021, the VeCC initiative in ISLHD was recognised as a finalist in the NSW Health “Transforming Patient Experience” awards for its innovative home-based model and improved self-management support.

The positive consumer experiences and clinical outcomes helped inform broader inclusion of VeCC and virtual wards in the NSW Virtual Care Strategy 2021–26, positioning virtual care as an integral part of future health service design.
